Instructions: Please complete Sections 1 and 3. (Leave Section 2 blank). Forms are also available on MHS Main Entrance door. Please return completed form to MHS Main Entrance door to envelope labeled “Completed DOT Forms”. Student will be notified via email when the form is signed and can be picked up. If you would like to be notified via phone, please provide a phone # to be contacted at.

- The Muskie Softball team defeated West Des Moines Valley 3-1 on Monday and are moving on to play Cedar Rapids Kennedy in a Semi-final game on Wednesday at 11am. If planning on attending, please review this info: https://ighsau.org/news/2020-state-softball-tournament-central/ Admission is $10.
Player send off will take place on Sunday, July 26th at 2pm in the MHS Bonnie Street Parking Lot. Go Muskies!

Venue Info: Harlan and Hazel Rogers Sports Complex in Fort Dodge, IA. Parking is available on-site at the facility at $10.00 per car. The address for the complex is 1628 Nelson Avenue, Fort Dodge, IA, 50501.
- MHS Band Camp is modified. We will meet August 3-7, 8am-12pm. Outside only. Come prepared with snacks and water bottles.
