The secretary will fax a request for student records to your child’s previous school.

For kindergarten only.

Medical considerations and medicines that need to be noted

If your student has a chronic health condition (i.e. seizures, allergies, asthma, etc.) there are additional forms available on our Health Services page.

If necessary, please provide court documents with information regarding the custody of the student or if the student is mentioned in a protection order.
